Transferring your records to another healthcare provider is simple. Contact us directly and submit a written request via email or in person at our clinic. For privacy and legal reasons, we require your signed authorization before we release any personal or medical information. Once we receive your request, we’ll process it promptly and securely share your records with the designated provider. If you have any questions or need help filling out the request form, our team will be happy to assist you.

In most cases, we provide a copy of your medical records free of charge, especially if they’re being sent directly to another healthcare provider for continuity of care. However, if you request a personal copy for your own records or ask for multiple printed copies, a small administrative fee may apply to cover printing and processing costs. We’ll always inform you in advance if any charges apply, so there are no surprises.

To ensure that a designated person has access to your medical records, you’ll need to complete a written authorization form specifying their name and relationship to you. This form grants us permission to share your personal health information with them, in accordance with privacy laws. You can request this form at our clinic or via email. Once it's filled out and signed, we’ll securely update your file to allow access to the authorized individual. You can change or revoke this permission at any time by submitting a new form.
